 To Enter
• Complete an entry form for each facility you wish to submit. Answer all questions as completely as possible. Please type your answers.
• Enter only one project per category. Each project requires a separate entry form.
• Submit at least four professional photographs (high resolution, at least 300 dpi) per project. If Photos are sent via CD, please label disc accordingly. Photos themselves should be labled with facility name and numbered or lettered to corresponding captions. Please provide photo credits and photo captions in a separate document.
• Submit at least one two-dimensional floor plan per project.
